What to know when its your time to get vaccinated.

Before you come, please:

1. Print and bring your completed 1st Dose Registration Form with you. (Registration form in Spanish)
2. (First Dose ONLY) If you scheduled your first dose appointment online through Novel Health, please bring your Novel Health account username and password to your first dose appointment. You will need your account information to schedule your second dose appointment.
3. If you are currently taking a blood thinner or have a bleeding disorder and have had a previous reaction to a vaccine or other injectable in the past OR have never had a vaccine/injectable in the past, please bring a doctors note with approval to receive the COVID-19 vaccine to your vaccine appointment.
4. Do not arrive more than 15 minutes early to your appointment time.
5. You should receive a text and email 24 hours before your appointment and you will need to complete the check in process BEFORE you show up to your appointment.

Second Dose Appointment Scheduling 

If you received your first dose from the Iredell County Health Department on or before March 1st, 2021, you WILL NOT need to schedule an appointment for your second dose.  You will receive an automated call from our agency informing you of when and where to return to receive your second dose.

Individuals who have already received their first dose, were given a ‘COVID-19 Vaccination Record Card’. The date on the back of that card is NOT an appointment, it is the first day the patient is eligible to receive their second dose:

• Moderna second dose can be administered as early as 28 days after the first dose

Health officials are asking individuals who received their first dose to monitor the COVID-19 Vaccine website for clinic dates that are on or after the date listed on the back of their ‘COVID-19 Vaccination Record Card’.

If you received your first dose with another provider, we encourage you to please return to the same provider for your second dose. Thank you
